Expensive Gun Failure

Watch until the end where you learn the reason for this failure. It’s quite common, actually, because of the similarity of ammunition case.
Check your ammunition. Load it by hand, and watch while you do.

Carelessness demands a price. Maybe not today, but soon.
Thankfully, no flesh was extracted. However, loosing meaty parts may be the single best way to remember the lesson.
[Side note: What is it these days guys looking like the Soggy Bottom Boys? Is it in remembrance of fighting with the Mujahedeen?]

About two years ago I was in my LGS and saw an AR-15 in the rack where they only keep customer’s guns that had a dust cover that from a distance looked a bit the worse for wear. When I asked the dealer about it, he just picked it up and handed it to me without saying a word.
It was a Wilson Combat in 5.56 that looked suspiciously like the one in the video. The dust cover was bowed, the BCG was sheared part way back from the front, and in this case the upper receiver was cracked at the rear edge. A cleaning rod down the barrel showed there still was a projectile in the barrel but the bolt was still locked in the breech, which prevented examining the casing.
The dealer sent it to Wilson and it came back a couple of months and $800 later with the diagnosis: 300 Blackout.
